Cytosolic Calcium Measurements in Renal Epithelial Cells by Flow Cytometry. Abstract. A variety of cellular processes, both physiological and pathophysiological, require or are governed by calcium, including exocytosis, mitochondrial function, cell death, cell metabolism and cell migration to name but a few. Cytosolic calcium is normally maintained at low nanomolar concentrations; rather it is found in high micromolar to millimolar concentrations in the endoplasmic reticulum, mitochondrial matrix and the extracellular compartment. Upon stimulation, a transient increase in cytosolic calcium serves to signal downstream events. Detecting changes in cytosolic calcium is normally performed using a live cell imaging set up with calcium binding dyes that exhibit either an increase in fluorescence intensity or a shift in the emission wavelength upon calcium binding. However, a live cell imaging set up is not freely accessible to all researchers. Alternative detection methods have been optimized for immunological cells with flow cytometry and for non- immunological adherent cells with a fluorescence microplate reader. Here, we describe an optimized, simple method for detecting changes in epithelial cells with flow cytometry using a single wavelength calcium binding dye. Adherent renal proximal tubule epithelial cells, which are normally difficult to load with dyes, were loaded with a fluorescent cell permeable calcium binding dye in the presence of probenecid, brought into suspension and calcium signals were monitored before and after addition of thapsigargin, tunicamycin and ionomycin. Keywords: Cellular Biology, Issue 9. Kidney, FACS, second messenger, proximal tubule, calcium indicators, probenecid, endoplasmic reticulum, ionomycin. Introduction. Calcium is an important second messenger responsible for the transmission of cellular physiological and pathophysiological signals. Cytosolic calcium concentrations are kept low through the activity of calcium pumps and calcium exchangers. The sarcoplasmic/endoplasmic reticulum calcium ATPase (SERCA) refills the endoplasmic reticulum (ER) calcium store as part of a “pump leak” system whereas the plasma membrane calcium ATPase (PMCA) extrudes calcium to the extracellular compartment, both in ATP- dependent manners. Physiological messengers, such as hormones or neurotransmitters, transmit their signals through G- protein coupled receptors, activation of phospholipase C, which hydrolyses phosphatidylinositol 4,5- bisphosphate (PIP2) in the plasma membrane to generate diacylglycerol and inositol triphosphate (IP3)3. While diacylglycerol remains in the plasma membrane, IP3 diffuses into the cytosol and binds to IP3 receptors (IP3. Rs), which are ligand activated calcium channels, found in the ER membrane and calcium is released from the ER luminal store culminating in an increase in cytosolic calcium concentrations. An alternative route for calcium to reach the cytosol is through calcium channels and exchangers present in the plasma membrane. Crosstalk between these two compartments have been described: calcium induced calcium release (CICR) where extracellular calcium induces calcium release from ER stores. SOCE) where emptying of the ER stores is sensed by STIM and causes opening of Orai calcium channels in the plasma membrane to promote refilling of the ER stores. Under pathophysiological conditions, the cellular calcium response is deregulated and cytosolic calcium increases in the absence of physiological stimulators. The calcium response may be affected in a number of ways: prolonged calcium signal, delayed calcium removal from the cytosol, depletion of calcium stores or localized changes in calcium. Furthermore, the mitochondria take on a central role in buffering and releasing calcium. Prolonged and/or supramaximal cytosolic calcium increase leads to cell death. In fact, calcium is more often than not involved in the cellular pathophysiological response and is a key event in a wide array of diseases, such as neurodegenerative disease, heart disease, cancer, bone disease and kidney disease, which are mainly associated with cell death and loss or alteration of organ or tissue function. Moreover, perturbation of calcium signals has been linked to cellular adaptation and changes in cellular functions and responses. Traditionally, calcium signals are measured with a negatively charged fluorescent calcium indicator that is loaded into cells in a cell permeable acetoxymethyl (AM) ester form. Once cleaved by cellular esterases, the fluorescent indicators remain within the intracellular compartment and increase in fluorescence intensity when calcium is bound. The most well known and used calcium indicator is the ratiometric Fura- 2 developed by Roger Tsien and colleagues. Calcium indicators with differing affinities for calcium allow different calcium pools to be monitored. Detection methods include live cell imaging, fluorescence microplate reader and flow cytometry. The relatively fast kinetics of calcium signals (usually within a few seconds to minutes) makes live cell imaging the best method for gaining the most information about characteristics of the calcium signal. Aside from the fast kinetics of calcium signals (within milliseconds), live cell imaging is a better method for studying cellular compartmentalization of calcium signaling within a single cell 1. Absolute calcium concentrations can be calculated by determining the minimum and maximum fluorescence of the calcium indicator by adding a calcium chelator and permeabilizing the cells, respectively, as described by Grynkiewicz et al. The use of flow cytometry to measure calcium signals was first developed in the 1. In adherent cells, live cell imaging provides the most information about calcium signaling, most importantly the kinetics, but requires a complicated setup comprising a fluorescence microscope, a perfusion system, maintenance of the cellular environment, such as temperature, and specialized microscope software for acquiring and analyzing data. Alternative methods such as a fluorescence microplate reader or pharmacological means through use of calcium chelators are incomparable in terms of gained information. Flow cytometry is becoming more widely used to monitor cytosolic calcium in adherent cells though, in the majority of studies, only end- point measurements are performed. Using the method initially developed by Gergely et al. B cells. 16, changes in cytosolic free calcium concentration by flow cytometry with real- time kinetics and monitoring the fluorescence intensity in individual cells from a sample population have been successfully measured in cancer epithelial cells and cancer stem cell hybrids. This method was further adapted for use in adherent epithelial cells, which are difficult to load with calcium indicators 1. Here, using an immortalized adherent epithelial cell line derived from the S1 segment of the rat kidney proximal tubule (WKPT- 0. Cl. 2)1. 9, we describe an optimized simplified method for measurement of changes in cytosolic calcium concentration by flow cytometry. Since many epithelial cells possess a number of organic transporters for anionic compounds, loading of cells with a calcium indicator can prove to be a challenge. To prevent the efflux of calcium indicators, probenecid, the prototypical inhibitor of organic anion transporters and originally developed to decrease renal excretion of penicillin. The cells are maintained in monolayers for calcium indicator loading, brought into suspension and calcium signals can be detected immediately after compound addition. Protocol. 1. Also included are host configuration details and instructions for installing guest virtual machines of different types, PCI device configuration and SR- IOV. What is in This Guide? This guide provides information on installing virtualization software and configuring guest machines on a Red Hat Enterprise Linux virtualization host. The initial chapters in this guide outline the prerequisites to enable a Red Hat Enterprise Linux host machine to deploy virtualization. System requirements, compatible hardware, support and product restrictions are covered in detail. Guest virtual machine installation is covered in detail starting from Chapter 6, Guest Virtual Machine Installation Overview, with procedures for installing fully virtualized Red Hat Enterprise Linux guests and Windows paravirtualized guests using virt- manager and virsh. More detailed information on networking, PCI device configuration, SR- IOV, KVM guest timing management, and troubleshooting help for libvirt and SR- IOV is included later in the guide. Chapter 2. System Requirements. This chapter lists system requirements for successfully running virtual machines, referred to as VMs on Red Hat Enterprise Linux 6. Virtualization is available for Red Hat Enterprise Linux 6 on the Intel 6. AMD6. 4 architecture. The KVM hypervisor is provided with Red Hat Enterprise Linux 6. Minimum system requirements. GB free disk space. GB of RAM. Recommended system requirements. One processor core or hyper- thread for the maximum number of virtualized CPUs in a guest virtual machine and one for the host. GB of RAM plus additional RAM for virtual machines. GB disk space for the host, plus the required disk space for each virtual machine. Most guest operating systems will require at least 6. GB of disk space, but the additional storage space required for each guest depends on its image format. For guest virtual machines using raw images, the guest's total required space (total for raw format) is equal to or greater than the sum of the space required by the guest's raw image files (images), the 6. GB space required by the host operating system (host), and the swap space that guest will require (swap). Equation 2. 1. Calculating required space for guest virtual machines using raw imagestotal for raw format = images + host + swap. For qcow images, you must also calculate the expected maximum storage requirements of the guest (total for qcow format), as qcow and qcow. To allow for this expansion, first multiply the expected maximum storage requirements of the guest (expected maximum guest storage) by 1. Equation 2. 2. Calculating required space for guest virtual machines using qcow imagestotal for qcow format = (expected maximum guest storage * 1. Guest virtual machine requirements are further outlined in the Red Hat Enterprise Linux 6 Virtualization Administration Guide in Chapter 6. Overcommitting with KVM. Calculating Swap Space. Using swap space can provide additional memory beyond the available physical memory. The swap partition is used for swapping underused memory to the hard drive to speed up memory performance. The default size of the swap partition is calculated from the physical RAM of the host. KVM Requirements. The KVM hypervisor requires. Intel processor with the Intel VT- x and Intel 6. AMD processor with the AMD- V and the AMD6. Refer to the Red Hat Enterprise Linux 6 Virtualization Administration Guide to determine if your processor has the virtualization extensions. Storage Support. The guest virtual machine storage methods are. LUNs. LVM partitions. NFS shared file systems. GFS2 clustered file systems. Fibre Channel- based LUNs, and. Fibre Channel over Ethernet (FCo. E). Chapter 3. KVM Guest Virtual Machine Compatibility. To verify whether your processor supports the virtualization extensions and for information on enabling the virtualization extensions if they are disabled, refer to the Red Hat Enterprise Linux Virtualization Administration Guide. DMA: National Correct Coding Initiative National Correct Coding Initiative (NCCI) *Please see the information below regarding “Additional Correct Coding Edits Implementation". Overview. In March 2. Patient Protection and Affordable Care Act of 2. H. R. 3. 59. 0) Section 6. Mandatory State Use of National Correct Coding Initiative (NCCI)] as amended by the Health Care and Education Recovery Act of 2. P. L. 1. 11- 1. 52), together referred to as the Affordable Care Act (ACA) was approved. This legislation requires all state Medicaid programs to incorporate “NCCI methodologies” in their claims processing systems. States have until March 3. NCCI mandate. NCCI is a program developed by CMS that consists of coding policies and edits that identify procedures and services performed by the same provider on the same date of service. This program has been in existence for Medicare since 1. Medicaid services. NCCI Components. There are two main components of the CMS mandate: NCCI – procedure- to- procedure edits for practitioners, ambulatory surgical centers, and outpatient hospital services that define pairs of HCPCS/CPT codes that should not be reported together. Medically Unlikely Edits (MUE) – these are units- of- service edits for practitioners, ambulatory surgical centers, outpatient hospital services, and durable medical equipment. This component defines for each HCPCS/CPT code the number of units of service that is unlikely to be correct. The purpose of the mandated edits is to prevent improper payments when a provider submits incorrect code combinations or to avoid payments of units in excess of what is medically unlikely. NCCI Implementation. To comply with the NCCI mandate, DMA will implement the two main components by March 3. The potential exists that these edits may impact claims payments for practitioners, ambulatory surgical centers, outpatient hospital services (only for drugs, high- tech images, ultrasounds, and labs as they are billed at a CPT/HCPCS code level), and durable medical equipment. As DMA progresses through the NCCI implementation, additional information will be published in the Medicaid Bulletin. New CCI and MUE EOBs will be reported on the provider’s Remittance and Status (RA) Report. Additional Correct Coding Edits Implementation. DMA plans to implement additional correct coding guidelines to enhance our current claim processing system. These new correct coding guidelines and edits will be nationally sourced by organizations such as the Centers for Medicare and Medicaid Services (CMS) and The American Medical Association (AMA). These edits will identify any inconsistencies with CPT, AMA, CMS and/or DMA policies and will deny the claim line. For example, the edits will ensure that: The appropriate procedure code is utilized based on age and gender of the patient. If a procedure code is submitted that requires a primary procedure code, DMA will verify that the primary procedure code has been submitted. Procedure codes are billed in the appropriate place of service as defined by AMA and/or CMS. For example, certain procedure codes are not permitted to be performed outside of an inpatient setting. Obstetric services including antepartum care, delivery, and postpartum care are billed appropriately according to CMS guidelines and DMA policy. The appropriate Evaluation and Management (E & M) codes are utilized for new patients and established patients. Certain services related to a surgical procedure are included in the payment of the global surgery package. These services would include E & M and related surgical procedures performed by the same physician for the same patient. Duplicate services are not submitted for the same provider, same patient for the same date of service. Descriptions of Planned Additional Correct Coding Edits. DMA will notify providers through the Medicaid Bulletin and other means when these additional correct coding edits are being implemented. Additional Information. Information regarding NCCI implementation is available in the Correct Coding Initiative Fact Sheet (PDF, 6. KB) and in the March 2. Seminar/Webinar Presentation (revised 3/2. PDF, 2. 5 MB). Click here for information on adjusting the number of units for submitted claims. Providers are encouraged to review published bulletin articles for information and updates on the status of the project and for information on provider training opportunities. National Correct Coding Initiative. Information pertaining to the NCCI mandate is also available on the CMS Medicaid NCCI Coding web page. The NCCI State Medicaid Director letter and the Frequently Asked Questions documents contain valuable information regarding this mandate. The comprehensive list of codes can be downloaded from the CMS NCCI and MUE Edits web page. The list is available by type of provider or service. Additional Correct Coding Edits. How do I connect to a remote Windows 7 desktop from a Linux machine? Although most administrators do their work within a Windows environment, there are times when an administrator will want to (or have to) connect to a remote Windows machine from within a Linux machine. Now most administrators are used to doing this from within Windows. But from within Linux it is a different story. There are plenty of Remote Desktop Protocol (RDP) tools available for Linux that allow you to connect to just about any platform, but for our purposes, we will be connecting to a Windows 7 desktop from an Ubuntu 9. But we'll make this a bit more complicated. Since "the cloud" is all the rage, we will make this Linux- to- Windows connection with the Windows 7 instance being served as a virtual machine from a different Ubuntu Server using Virtual. Box. This blog post is also available in PDF format in a free Tech. Republic download. In order to make that connection a little easier to understand, let's map it out with a diagram (Figure A). Figure AThe Windows 7 desktop is served up by the Ubuntu Server via Virtual. Box 3. 0. Now that you can see a visual representation, the connection should be easy to figure out. For the purposes of this document, I am going to assume: You have a working Ubuntu Server. You have Virtual. Box working on your Ubuntu Server. You have Windows 7 installed on Virtual. Box. You have a working Ubuntu desktop. The above issues are simple to resolve. If you don’t know what Proxy Server is then it is a redirect via an IP to the external site, usually used for anonymous surfing. If the IP / Proxy is down, or if it.There are a few others that might not be so obvious, so we'll discuss those here. Bridging your network. If you go with the default settings on your Windows 7 Virtual Machine, you will not be able to access that desktop because the IP address will be handled via NAT. Instead you need to use a bridged connection so that your virtual machine will be on your LAN IP address scheme. To do this, you need to close the Windows 7 virtual machine. You can't just "pause" the instance; you actually have to close the session (as if you were going to shut down a regular Windows machine) to access the configuration of the VM. Once you have that VM closed, go to Virtual. Box, select the Windows 7 VM, and then click the Settings button. Click on the Network section and then, from the Attached To drop- down menu (Figure B), select Bridged Adapter. Click OK to Apply this setting and dismiss the window. Figure BIf you find networking does not work on your virtual machine, you might have to change the Adapter type. Now go back to the main Virtual. Box window and start up your virtual machine. You should find, once Windows 7 boots, that it is now a part of your LAN IP address scheme. Enable RDPBefore you try to connect to your Windows 7 desktop via RDP, you have to enable the service. To do this, follow these steps from within your Windows 7 virtual machine: Click on the Start Menu. Right- click on the Computer entry. Select Properties. Click on the Remote settings entry. Make sure that both the Allow Remote Assistance Connections to This Computer and Allow Computers Running Any Version of Remote Desktop are checked. After you take care of those settings, click OK to set them. Linux RDP client. One of the more reliable and user- friendly of the Linux RDP clients is Terminal Server Client (tsclient). By default, this should be installed on your Ubuntu desktop. If it isn't, you can go to the Ubuntu Software Center, search for "terminal server" (no quotes), click on the Terminal Server Client entry, click the right- pointing arrow, and finally click Install. Once that software is installed, you will find it in the Internet sub- menu of the Applications menu. Start that up, and you will see the main tsclient window (Figure C). Figure CAs you can see, it takes very little configuration to connect to your Windows 7 desktop. Believe it or not, very little effort is required to make the connection from Linux. All you need to do is enter: The IP address of the Windows desktop you want to connect to. The user name that you will log in as. The password for the user. After you enter that information, click Connect and a new window will open, running a remote instance of your Windows 7 desktop (Figure D). Figure DOur virtual machine instance of Windows 7 is running, via RDP, on an Ubuntu desktop machine. Final thoughts. This type of setup should illustrate for you how versatile RDP can be. Doing remote work on a Windows 7 machine does not have to be limited to using another Windows 7 machine. Now you know that administering a Windows 7 machine can be done remotely from a Linux desktop. |
